Who May Avail of the Service? Are presently enrolled over-age pupils and need to upgrade their academic level.
What are the Requirements?
Birth Certificate Latest Card or Form 137 (original and photocopy) 2 pcs. 1x1 ID picture Registration Fee - Regular =Php 50 An Individual is qualified to take the PEPT If he/she completed the grade as shown below:
Grade Age at the time of registration Grade 1 at least 8 yrs .old Grade 2 at least 9 yrs. old Grade 3 at least 10 yrs. old Grade 4 at least 11 yrs. old Grade 5 at least 12 yrs. old Grade 6 at least 13 yrs. old

Kindergarten and Elementary - Early Registration: every last Saturday of January - Two weeks before the start of the School Year

v Kindergarten age - 5 years old as of Oct 31 of the current year v Elementary age - 6 -11years old
Requirements for Enrollee:
1.Kindergarten and Grade I v Birth Certificate (original and xerox copy) 2.Elementary – Grade II –VI v Report Card (Form 138) 3.Transferees v Report Card (Form 138) with LRN v Copy of Bith Certificate v Form 137-E with LRN v Certificate of Enrollment (In case of inavailability of Form138)

Who may Avail of the Service? v Recruitment: Acceptance and Evaluation of Teacher’s Application Ø Applicants who hold a valid of registration/ professional license as a teacher from the Professional Regulation Commission (PRC) seeking to be appointed as Teacher Ø Qualified applicant who is with a valid professional license issued by the Professional Regulation Commission and who meets the requirements in the Evaluation Selection Guidelines applied by the school Ø Bonafide resident of Barangay Tenejero for at least six months who declared himself/ herself in the Personal Data Sheet

What are the Requirements?
v Recruitment: Ø Application Letter Ø CSC Form 212 in 2 copies with the latest 2x2 ID picture Ø Certified photocopy of PRC Certificate of Registration /License Ø Certified photocopy of ratings obtained in the Licensure Examination for Teachers/ Professional Board Examination for Teacher. Ø Certified copies of Transcript of Records for baccalaureate course Ø Certificates of specialized training, if any
How to Avail of the Service?

Who may Avail of the Service? Any person may file an administrative complaint against teaching and non-teaching personnel of the school
Fees: NONE What are the Requirements?
1. Full name and address of the complainant. 2. Full name and address of the person complained of, as well as his position in the school. 3. A narration of the relevant and material facts which show the acts or omissions as allegedly committed by the person. 4. Certified true copies of documentary evidence and affidavits of his witnesses, if any 5. Certificate of Non-Forum Shopping 6. Complaint shall be under oath
